PLEASE NOTE:

1) This form does not confirm your order.  We will be in contact

with final pricing and confirmation.

2) At least a two-week notice is highly recommended as we cannot guarantee

product and availability.

3) A 50% deposit will be required to confirm your order and date.

Deposits are only refundable up to 7 days before the event date.

4) Pick-up and Delivery available if within 20 miles from Troy, MI.

Delivery fees may apply.
